Our goal is to provide comforting, compassionate care to those in need of specialized care while maintaining their dignity and respect.

Our registered nurses (RN), licensed practical nurses (LPN), and certified nursing assistants (CNA) provides around-the-clock nursing care for those with complex medical conditions who need assistance with activities of daily living – getting out of bed, bathing, using the toilet, dressing, walking, or eating.

Depending on the needs of each individual, other team members may come into play including physical and occupational therapists; speech language pathologists; registered dietitian, and social workers and coordinators. Together, these professionals create a personalized care plan based on the individual’s strengths, needs and abilities.

Our experienced team provides specialized care for those with:

  • Chronic medical conditions
  • Effects of a Stroke (CVA)
  • Neurological and neuromuscular disorders
  • Chronic obstructive pulmonary disease (COPD)
  • Congestive heart failure (CHF)
  • Permanent disabilities including amputations
  • Back injuries
  • Acute or prolonged illness

  • Wound management
  • Diabetic management
  • Gastrostomy tube services
  • Adaptive equipment recommendations
  • Hydration program
  • Restorative nursing programs
  • Physical, occupational and speech therapy
  • Treatment for swallowing disorders (Dysphasia Therapy)

Care Plan Meetings Providing Peace of Mind

Within the first week of residing here, a Care Plan Meeting is held to further discuss and clarify each individual’s goals, and for residents and their families to ask questions regarding clinical needs and care. The resident representative, family and/or patient advocate are welcome to attend with resident’s approval. Care Plan Meetings are then held every three months or as requested during someone’s stay. If questions or concerns arise, we encourage individuals to contact any of our team members at any time.
